CRITICAL ALLERGIES & REACTIONS
⚠️ ANAPHYLAXIS RISK: – Sulfa drugs – hives, low BP, mild anaphylaxis – Symbicort (budesonide) – mild anaphylaxis – Gold Sodium Thiosulfate
CONTACT ALLERGIES: – Formaldehyde & formaldehyde-releasing preservatives – Quaternium compounds – Methylchloroisothiazolinone/Methylisothiazolinone – Parabens
SENSITIVITIES: – Gluten, aspirin, fragrances
IgE ALLERGIES: – Insect bites – Respiratory allergens: cats (Class 3), dust mites, molds
CURRENT ACTIVE CONDITIONS
- Myasthenia Gravis (MG) — Seronegative (AChR neg, MuSK neg, LRP4 neg). Dx clinically by Dr. Bravo (neurologist, Puerto Vallarta) via positive response to Mestinon. Started Mestinon 15mg QID January 2026 with positive clinical response. Retrospective probable episodes: May 2017 (exertional weakness + respiratory difficulty, Yosemite) and November 2024 (severe exacerbation with bulbar involvement, Puerto Peñasco). ⚠️ See MG Medication Warning section below.
- Mast Cell Activation Syndrome (MCAS) – Major flare July-Aug 2025, now improving
- Lyme Disease – Diagnosed Feb 2025 (IgG positive), contracted early 1980s
- Epstein-Barr Virus – Diagnosed Feb 2025 (IgG positive 14.49 S/CO)
- Mycotoxin Exposure – HIGH levels: Ochratoxin A (26.46), Citrinin (329.78), Mycophenolic Acid (91.60)
- Non-Alcoholic Fatty Liver Disease (NAFLD) – Diagnosed (year unknown), monitored with labs/imaging
- Obesity – Currently on treatment protocol
- Major Depressive Disorder (MDD) – Diagnosed age 54 (2025), stable on Lexapro since mid-40s, likely had condition since childhood
- Generalized Anxiety Disorder – Diagnosed 30s, stable on Lexapro
- Asthma – Well-controlled
- Fibromyalgia
- Venous insufficiency – S/P bilateral ablation 2019
- Baseline temperature – Waking oral temperature varies 95.8–97.6°F based on ambient sleeping conditions and does not reflect standard population norms; Oura Ring skin temperature deviation is the more reliable baseline indicator, with a personal resting range of 0.0 to +0.1°C. A deviation exceeding +0.25°C should be considered clinically significant regardless of absolute oral temperature.

RECENT MCAS FLARE (July-Aug 2025)
TRIGGER EVENT: Montana visit – exposure to Lime Away/Tide cleaning products (contain formaldehyde compounds)
ESCALATING FACTORS: – New unwashed blanket (fabric treatments) – Treated lumber smoke exposure (campground) – Wildfire smoke – Chemical retail environments – Heat/sun exposure
SYMPTOMS DURING FLARE: – Severe forearm dermatitis at exposure site – Heat/sun intolerance with vesicles – IBS flare – Neurological: “beam” sensation head-to-chest, eye twitching – Respiratory: shallow breathing, pulse ox 90% – Contact sensitivity (couldn’t wear watch/jewelry) – Stabbing nerve pains
CURRENT STATUS (Sept 2025): – Significant improvement after prednisone taper (Aug 26-30) – Weight dropped 2.5kg in one week (inflammation reduction) – Can now wear Garmin watch again – Still avoiding known triggers – Managing with H1/H2 antihistamines PRN
KEY MCAS TRIGGER PATTERNS
CHEMICAL TRIGGERS: – Formaldehyde-containing products – Fragrances/scented products – Cleaning chemicals – Treated lumber smoke – Large retail environments
FOOD TRIGGERS: – Nightshades (tomatoes, peppers) – High-histamine foods – Processed foods with preservatives – Liquid smoke
PHYSICAL TRIGGERS: – Heat/sun exposure – Dermatographism (pressure/friction) – Stress/poor sleep
RECOVERY FACTORS: – Clean air environments – Cool temperatures – Adequate sleep – H1/H2 antihistamine protocols
⚠️ MYASTHENIA GRAVIS — MEDICATION CONTRAINDICATIONS
Patient has seronegative MG. The following drug classes can trigger MG exacerbation or myasthenic crisis and must be avoided or used only with neurologist approval:
AVOID: – Fluoroquinolones (ciprofloxacin, levofloxacin, moxifloxacin) – Macrolides (azithromycin, clarithromycin, erythromycin) – Aminoglycosides – Beta-blockers – Magnesium (IV) – Neuromuscular blocking agents – Quinine/quinidine – Chloroquine/hydroxychloroquine
USE WITH CAUTION / NEUROLOGIST APPROVAL REQUIRED: – Any new antibiotic – Statins – Botulinum toxin – Contrast agents
PRIOR EXPOSURE WITHOUT MG DIAGNOSIS: – November 2024: received levofloxacin + azithromycin during probable MG exacerbation (pre-diagnosis)
PROBABLE RETROSPECTIVE MG EPISODES (PRE-DIAGNOSIS)
May 2017 — Yosemite National Park During a sustained hike, experienced severe exertional weakness with heavy legs and significant breathing difficulty. Required completing hike under duress without ability to rest and recover. No medical treatment sought at the time. In retrospect, consistent with MG neuromuscular junction fatigue under sustained exertion, heat, and altitude — all known MG exacerbation triggers.
November 10, 2024 — Puerto Peñasco, Sonora Following respiratory viral illness (bronchitis), developed acute crisis: extreme weakness, difficulty speaking (bulbar involvement), BP 104/50, pulse 41, inability to retain any fluids, vomiting bile. WBC 19,000 with 94% neutrophils on labs at Clinica San Jose Emergencias. Probable MG exacerbation with bulbar involvement triggered by systemic infection. Received two antibiotics subsequently identified as contraindicated in MG (levofloxacin, azithromycin). UTI diagnosis uncertain — urine sample showed abundant epithelial cells consistent with contamination.
